By providing Your email to us, You consent to receiving electronic communications, including, but not limited to, service and promotional updates. If You provide Your mobile telephone number, You consent to receiving text (SMS) messages from us at that telephone number. We will use Your PII to set up Your Urgently Health account, to process Your service requests, to communicate with You regarding to Your account, and to provide You with other information and communications that may be of interest to You, subject to applicable laws and the terms of this Privacy Policy. We may also share Your PII with third parties, including Your health care provider and suppliers that We engage with to provide Our Services.

Protected Health Information and the Health Insurance Portability and Accountability Act (HIPAA)
By using Our Services to visit or otherwise communicate with a healthcare provider, We will provide some of the information You have provided to Urgently Health to that healthcare provider. Under HIPAA, protected health information is individually identifiable information relating to the past, present, or future health status of an individual that is created, collected, or transmitted, or maintained by a HIPAA-covered entity in relation to the provision of healthcare, payment for healthcare services, or use in healthcare operations. Some of this health information includes, but is not limited to, diagnoses, treatment information, medical test results, and prescription information. Urgently Health may also collect PHI from Your healthcare provider or other authorized third parties when they submit such information through Our Services.

Urgently Health will only use or disclose PHI as required or permitted by applicable federal and state laws, including HIPAA, and in accordance with any contractual obligations that We may have with Your healthcare provider. If We share Your PHI with any third parties, including, but not limited to, Our suppliers, only the minimum necessary information will be shared to properly provide and maintain Our Services, requiring that these third parties agree to use and disclose Your PHI only as required or permitted by applicable laws.

Health Care Provider Search and Visitation Information
To search for healthcare providers using Our Services, Urgently Health requires the following: access to Your location, either by allowing us to detect Your geographical location via Your device’s location settings or by manually submitting Your location, and the reason for Your visit. To submit a visit with an available healthcare provider, You will need to also submit PII, discussed in the Personally Identifiable Information section of this Privacy Policy.

Account and Profile Information
To create an Urgently Health account using Our platform, You will need to submit Your first and last name, mobile telephone number, and email address. The information You submit when first creating an account will be used for Your Urgently Health profile. You may also choose to include additional information in Your profile, including, but not limited to, health histories (surgeries, allergies, etc.), insurance information, primary care physician information, and lifestyle information.

Account Activity
Urgently Health collects information on how You use Your account. Data that may be collected includes, but is not limited to, health care providers You choose to visit and the reasons for those visits.

Location Information
Urgently Health may access Your location with Your consent to improve and provide better Services to You. You may choose to allow us to access Your location by granting us access to Your location when prompted on Our platform or through Your device’s location settings. We may also use GPS and IP addresses. Additionally, Urgently Health uses the Google Maps/Google Earth API(s). Use of this location feature is also subject to Google’s privacy policy, which can be found at http://www.google.com/policies/privacy.

Data Collected by Our Servers
The servers used to run Our Services may collect data pertaining to You, Your equipment, software, and methods You use to access the internet and Our Services. Some of the data We collect includes, but is not limited to, Your internet protocol, Your internet service provider, Your geographic location, Your browser type, and Your browsing habits while using Our Services. Urgently Health may use this data, but is not limited to, ensuring server and service functionality, to generate statistical information, including data analysis and usage patterns, to maintain and enhance digital security, and to make improvements to Our Services.
